We are currently seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to provide in-home assessments, care coordination, and clinical support for Medicare-age patients throughout Wake County, North Carolina. This is an opportunity to work directly with seniors in their homes, helping improve health outcomes, reduce avoidable hospitalizations, and support patients with complex medical and social needs.

Conduct comprehensive in-home health assessments for Medicare Advantage members

Perform medication reviews, health screenings, and risk assessments

Identify medical, behavioral, and social factors that may impact patient outcomes

Educate patients and caregivers on disease management, medications, and preventive care

Develop and support individualized care plans based on patient needs

Coordinate care with primary care providers, specialists, and ancillary healthcare partners

Collaborate with physicians, nurses, social workers, care coordinators, and health coaches

Identify care gaps and recommend interventions to improve quality outcomes

Document patient encounters and care recommendations accurately within the electronic medical record

Support initiatives focused on reducing hospitalizations, improving chronic disease management, and enhancing patient quality of life

Master's Degree from an accredited Nurse Practitioner or Physician Assistant program

Active North Carolina Nurse Practitioner or Physician Assistant license

Active board certification as a Nurse Practitioner or Physician Assistant

Active DEA Registration

Strong communication and relationship-building skills

Ability to work independently in a fast-paced field-based environment

Home health, home-based primary care, or community-based care experience

Previous electronic medical record (EMR) experience

Experience working within value-based care or managed care environments
