Position Summary The wound care nurse will specialize in assessing and treating patients with complex wounds, working closely with other members of the patient care team to ensure that treatment plans are followed correctly to prevent future infection or injury.

Works collaboratively with patient care team to improve skin, wound, and ostomy health throughout facility

Provides wound and ostomy care treatments

Facilitates wound rounds, to complete dressing changes and wound care

Responsible for monitoring and follow up of all ulcers and incisions with appropriate documentation and communication to physician and family

Assesses weekly nutrition/hydration of patients with ulcers and incisions, evaluate and initiate additional nutritional intake as required

Competencies Demonstrates the ability to implement and evaluate the effectiveness of interventions in the plan of care and to recognize the need to reassess as required

Read, write, speak, and understand the English language

Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations and guidelines that pertain to nursing care facilities and infection prevention and control practices to include standard/universal precautions.

This role requires regular walking to various locations around the facility. Position Type and Expected Hours of Work This is a full-time position in a post-acute setting that is open 24 hours a day, 365 days a year. Weekend and holiday shifts will be required

HIPAA Team members are required to adhere to policies and procedures implementing HIPAA requirements for the privacy and security of protected health information. Team members are permitted to use and/or disclose only minimum amount of Protected Health Information necessary to complete assigned tasks. Reports all suspected violation of company’s HIPAA policies or procedures to the Administrator.

Interpersonal skills necessary in order to provide effective leadership, maintain a variety of internal and external contacts, and work cooperatively with a variety of facilities, managers, executives, and team members

Analytical abilities necessary in order to evaluate delivery of care systems; Ability to communicate effectively with internal and external customers at all levels of the organization

Proficient computer and Microsoft Application skills. Ability to work with a culturally diverse resident and employee population

Proficient in the English language. Excellent communication (verbal and written), customer service and interpersonal skills

Must be able to successfully prioritize workload, demonstrate initiative, and analyze situations to make sound decisions based on available data

Self-directing with the ability to work with minimal direct supervision

Required Education and Experience Valid Nursing License

A minimum of one (1) year experience in a healthcare setting

Schedule Part-time

Sunday, Tuesday and Thursday- 8hours
