As a team member of our Senior Community Care (SCC) team, we work to provide care to patients at home, nursing homes and assisted living for senior housing. You will be part of the primary care team collaborating with all interdisciplinary team members and will support multiple providers. The salary for this role will range from $60,200 to $107,400 annually based on full-time employment.
