Registered Nurse (RN)Physicians Choice Home Health is hiring a full-time or part-time/per diem Registered Nurse (RN) to join our Home Health team, serving patients in the South Bay of Los Angeles. In this role, you will provide high-quality nursing services to patients in the comfort of their own homes, helping them manage their care and regain independence. You will oversee a supporting Licensed Vocational Nurse and work closely with physicians, therapists, and a supportive clinical team to coordinate care and support patients and their families.Physicians Choice is part of Obran Cooperative, a worker-owned healthcare organization where clinicians have a real voice, shared ownership, and a long-term stake in the success of the company.What You'll DoProvide in-home nursing visits according to the physician's plan of careAssess patients' health status, including condition, history, and home environmentDevelop and update individualized care plans based on patient needsAdminister medications and treatments as prescribedEducate patients and caregivers on disease management and home careMonitor patient progress and communicate changes to the physician and care teamDocument visits and care in a timely mannerCoordinate care with therapists, aides, and other team membersServe as case manager for assigned patients and support discharge planning
