Registered Nurse - Hospital - Home Health Care/Hospice

Home Health Registered Nurse (RN)

Seeking an experienced and compassionate Registered Nurse (RN) to provide high-quality care to patients in a home health setting. This role is responsible for managing a caseload of patients while delivering skilled nursing services, ensuring continuity of care, and collaborating closely with an interdisciplinary team. The ideal candidate brings strong home health experience and a patient-centered approach to care delivery.

Schedule: Monday through Friday, 8:00 AM to 4:30 PM

Pay Rate: $51 to $53 per hour

On Call/Weekend Requirement: None

Mileage Reimbursement: Yes, between patient visits

Manage and coordinate care for a designated group of home health patients

Perform patient admissions, recertifications, and discharges in accordance with regulatory and agency standards

Develop, implement, and update individualized care plans

Collaborate with physicians, therapists, social workers, and other interdisciplinary team members

Provide patient and family education related to care plans, medications, and disease management

Maintain compliance with agency policies, procedures, and regulatory requirements

Active Registered Nurse (RN) license

Proven home health experience

Strong clinical assessment and case management skills

Experience with MatrixCare and Homecare Homebase (HCHB) systems
