Description

Conduct thorough evaluations of patients in both inpatient and outpatient environments, crafting individualized treatment plans to facilitate recovery.
Deliver physical therapy interventions that focus on enhancing mobility, strength, balance, and overall functionality.
Work collaboratively with nurses, physicians, case managers, and other rehabilitation professionals to ensure optimal patient outcomes.
Educate patients and their families on therapeutic goals, discharge protocols, and exercises to continue their journey at home.
Document evaluations, treatments, and patient progress in adherence to facility regulations and guidelines.
Engage in care team rounds and interdisciplinary meetings to promote coordinated and holistic patient care.
Education
~ Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited educational institution.

Certifications
Current state licensure as a Physical Therapist .
CPR certification preferred.
Specialty certifications (such as neurological or geriatric) are a plus but not required.
Skills
Previous hands-on experience is strongly preferred.
Proficient in treating patients with complex medical and mobility challenges.
Excellent communication and teamwork skills.
Comfortable navigating digital charting and clinical documentation systems.
