Homecare CHHA RN - Brooklyn, NY
Salary: $115,000 - $120,000 per year. Sign‑on bonus: $15,000. Position within a long‑standing NYC Metro Healthcare organization offering hospice, palliative, rehabilitation, and skilled nursing services.
Overview
Homecare Coordinator of Care / CHHA RN will oversee the process of patient recovery from illness, injury, or surgery, conducting daily patient visits and ensuring comprehensive care coordination.
Location
Brooklyn, NY. Monday through Friday, 8:30 am to 5:00 pm; 5-6 patient visits per day.
Responsibilities
Implement and evaluate home care needs of patients, including assessment of safe living environment and support systems.
Communicate care plans between physician, physical therapist, social worker, and home health aide services.
Reconcile patient medications with the physician.
Assess patient and caregiver medication knowledge, instruct on usage, side effects, dosage, routes, and frequencies.
Evaluate and implement interventions for wounds, disease signs, complications.
Educate patients and caregivers on disease treatment and management tools.
Place wound vacs and Unna Boots as instructed by the physician.
Complete and submit OASIS assessments, reassessments, transfers, resumption of care, discharges, and significant condition changes within agency‑defined time frames.
Review patient insurance information to coordinate activities for each care plan.
Share pertinent findings with family and professional caregivers.
Enter all patient visit documentation into the KanTime EMR.
Qualifications
Registered Nurse with active New York State Nursing license in good standing.
Homecare / CHHA experience preferred.
OASIS experience highly preferred.
Must have a vehicle for patient visits.
