Form Health is a virtual obesity medicine clinic delivering multi-disciplinary evidence-based obesity treatment through telemedicine. Obesity impacts more than 40% of the US adult population, and although historically only about 1% of patients received medical treatment for their disease, the field of Obesity Medicine is entering a period of rapid growth. Form Health provides high-quality expert care and leverages technology to enhance the patient experience. All Form Health patients work closely with their physician-led care team, which also includes advanced practice professionals and Registered Dietitians. Through our proprietary mobile app patients engage in regular video visits, as well as text messaging, photo journaling, digital data transmission, and customized educational materials. Our mission is to empower patients and be leaders in Obesity Medicine driving impact at a national scale. We are seeking obesity medicine physicians to provide longitudinal telehealth care for patients with obesity, and to collaborate with advanced practice professionals and registered dietitians towards exceptional team-based care. Work is remote, and schedules can be flexible.

We are currently accepting applications for a FTE (40 hours per week) Obesity Medicine Physician or Endocrinologist opportunity, who are licensed in NEW YORK (NY), VIRGINIA (VA), or MASSACHUSETTS (MA).

This role will include participation in a rotating after hours call schedule with Form Health clinical leadership and will have a reduced clinical schedule the weeks of on-call coverage.

This is a Full Time (W2, 40hrs/week) position based in a remote, telemedicine clinical setting. The physician will work closely with a supportive care team and have protected time for administrative duties.

The physician will have their own panel and will collaborate with Registered Dietitians.

At this time, this role does not collaborate with APPs. In the future, this physician role will be responsible for chart reviews and conduct return visits for APP patients every third visit to ensure quality and continuity of care.

Provide treatment planning and medical management support for complex cases or escalated concerns.

Assist in the professional development of APPs by fostering clinical growth, offering feedback, and supporting continuing education. This role could include APP collaboration and supervision responsibilities.

Participate in team-based care initiatives and contribute to a culture of collaboration and high-quality care.

Attend relevant departmental meetings and contribute to quality improvement efforts within the obesity medicine program.

must be an American Board of Obesity Medicine (ABOM) diplomate or board certified in Endocrinology

Applicants must either already hold a valid Letter of Qualification (LOQ) through the Interstate Medical Licensure Compact (IMLC) or meet all prerequisites to rapidly secure one upon hire

Are committed to the highest quality of medical care and an evidence-based approach to obesity management

Have prior experience conducting telemedicine visits and who have a great "web-side" manner

Have prior experience working collaboratively with advanced practice professionals and registered dietitians

Bilingual candidates with medical fluency in Spanish are highly preferred
