LPN Nurse Home Care Shifts (Licensed Practical Nurse)

Nurse Supervisor/Wound Nurse (AM Shift) - Eden Vista Hoffman Estates, a senior living facility and part of Vista Senior Living Management. This role involves leading nursing services, financial management, and effective operations for residents and staff.

Develop, implement, and oversee nursing services, policies, and procedures in compliance with federal, state, and local regulations.

Lead and manage the facility's wound care program, including comprehensive assessments, treatment planning, documentation, and ongoing monitoring of residents with wounds.

Evaluate the effectiveness of wound care interventions and adjust treatment plans as needed to promote healing and prevent complications.

Conduct routine nursing rounds throughout the facility, including at the beginning and end of each shift.

Ensure residents receive safe, high-quality care while protecting their rights, dignity, and well-being.

Collaborate with physicians, therapists, and the interdisciplinary team to coordinate resident care.

Participate in care planning meetings, quality improvement initiatives, policy development, and administrative decision-making.

Maintain accurate clinical documentation and ensure compliance with regulatory standards.

Current, active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license.

Graduate of an accredited nursing program.

3-5 years of wound care experience required.

Wound Care Certification required.

Minimum of 3 years of experience in a long-term care, skilled nursing, or assisted living setting preferred.

Working knowledge of state survey regulations, Medicare, and Medicaid requirements.

Proficiency with Microsoft Office and electronic medical records.
