Position Overview
We are seeking a dedicated and experienced Physical Therapist Assistant (PTA) to join our home health team in the following areas: Danville, Martinez, Moraga, Alamo, Clayton, Concord, Lafayette, Orinda, Pleasant Hill, Walnut Creek, Pittsburg, Discovery Bay, Byron, Antioch, Brentwood, Oakley, Bethel Island, and Bay Point. This position is ideal for professionals with a strong passion for providing quality home health care to patients.
Key Responsibilities
Deliver one-on-one patient care in accordance with established treatment plans.
Visit 5-6 patients daily to meet productivity standards (27 productivity points standard).
Maintain timely communication and thorough documentation of patient interactions.
Coordinate with other healthcare professionals to ensure comprehensive patient care.
Utilize provided documentation tools and ensure their return upon completion of assignments.
Manage transportation of self to patient locations within a 50+ mile radius when necessary.
Schedule and Availability
Shift: Day 5x8-Hour (08:00 AM - 04:30 PM)
Guaranteed hours: 36 hours weekly, ramping up in the first two weeks.
Weekends available for patient visits to meet productivity requirements.
No on-call duties required.
Qualifications
Minimum of 1 year of home health experience required.
Current California PTA license is mandatory.
BLS certification required.
Skills and Expertise
Experience working with diverse patient populations including those with cognitive disorders and various physical conditions.
Familiarity with common medical equipment such as crutches, gait belts, walkers, and prosthetics.
Knowledge of rehabilitation for conditions like hip fractures, spinal surgeries, and post-stroke rehabilitation.
Additional Information
Dress Code: Any color scrubs are acceptable.
Free parking available at patient locations.
Mileage reimbursement will be provided at the IRS standard rate.
