Elara Caring is looking for a compassionate, dependable RN with home health experience who’s ready to help patients and families navigate care with confidence. You’ll build relationships, support each patient’s goals, and work closely with a team committed to quality care.
Visit patients in their homes to assess their health and care needs.
Complete initial, ongoing, and focused nursing assessments.
Develop and update plans of care with patients, families, physicians, and the care team.
Provide nursing care according to physician orders and the patient’s plan of care.
Monitor patient conditions, recognize changes, and communicate concerns to the care team.
Document visits and care accurately and promptly in Homecare Homebase (HCHB).
Help ensure each patient receives safe, coordinated, and compassionate care.
Current Registered Nurse license in the state of practice.
Graduation from an accredited nursing program.
At least one year of nursing experience in a clinical setting.
Home health experience and familiarity with Homecare Homebase (HCHB).
Valid driver’s license with an acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.
Ability to frequently lift, push, pull, and support up to 50 pounds, including assisting with patient transfers and moving equipment.
Typically, 3–4 days per week, with flexibility based on your availability.
