Image Strategic Partners
Image Strategic Partners is seeking a compassionate Hospice Care Aide to support patient care in Pueblo, CO. The role follows written care plans and assists with daily living tasks to ensure comfort and safety in the home setting.
You will work with RNs and clinicians to implement the hospice plan, document care, and help coordinate patient care conferences. A high school diploma or GED and appropriate state licenses are required, with benefits including medical, PTO, and mileage reimbursement.
