Manages the patient and family experience through critical thinking, teamwork, effective communication, and nursing judgment, knowledge, and skills.
May lead interdisciplinary case conferences in outpatient special care settings.
Adheres to all local, state, and federal regulations, codes, policies, and procedures to ensure privacy and safety, helping deliver optimal patient care.
Actively manages the patient and family plan of care to achieve optimal health goals.
Utilizes resources to deliver quality care in a safe, efficient, and cost-effective manner.
Coordinates the activities of the healthcare team in a professional manner to promote a positive work environment.
Provides care to patients and families that is collaborative, evidence-based, and meets national and facility standards.
Ensures the best possible patient and family experience while promoting a positive relationship with the community.
Possesses knowledge of population-specific learning principles related to patient teaching.
Independently practices and prioritizes multiple demands in a fast-paced environment.
Possesses good interpersonal and communication skills, both oral and written, and effective teamwork skills.
