Phoenix Home Care and Hospice
Phoenix Home Care and Hospice is seeking an Admissions RN for a full-time, Monday - Friday days position in Kansas City, MO. You will be the first contact between a patient and hospital, performing initial assessments, interpreting diagnostics, and formulating an individualized plan of care per physician orders.
The role involves collaborating with Nurses, Physical, Speech, and Occupational Therapists, and supporting patients with education and ongoing care in home healthcare settings.
