RN - Registered Nurse Ambulatory Care -Full Time

Join our team as a day shift, full‑time RN Care Navigator at INTEGRIS Health Medical Group South Pulmonary Clinic in Oklahoma City, OK.

Follows patients during their hospital stay and continues following post‑discharge to facilitate self‑management and avoid preventable readmissions.

Works closely with a multidisciplinary team to develop and implement the patient’s plan of care.

Ensures patients discharged from the hospital have a follow‑up appointment with their provider and reviews any unmet needs prior to the appointment.

Oversees hand‑off of patients to post‑acute facility, agency and primary acute provider, maintaining contact and providing interventions as needed.

Plans and conducts post‑discharge follow‑up phone calls with patients or caregivers to monitor progress, provide education and assist in decision‑making regarding self‑care.

Provides education, tools and training that align with patient needs and willingness to learn to help patients and caregivers make informed decisions.

Utilizes motivational interviewing techniques and other engagement strategies to facilitate adoption and adherence of care plans.

Ensures open communication with providers, patients and caregivers to coordinate services within the organization, at outside facilities, at home and physician offices to promote care and maximize satisfaction.

When working in clinical areas, potential exposure to infections, blood and body fluids, electrical equipment, chemicals. Current licensure as a Registered Nurse (RN) in the State of Oklahoma or a multistate license from a Nurse Licensure Compact (NLC) member state.

Bachelor’s degree in nursing preferred.

Case‑management experience preferred.

Computer experience required; Windows and Excel preferred.

Driver: this job requires operating a company‑owned vehicle or a personal vehicle with a current Oklahoma State driver’s license and acceptable driving record.
