Weekend Infusion & Home Health RN

We’re looking for nurses who enjoy working with patients and caregivers to improve the health and quality of life of our community to join our Weekend team.

Unlike most home care agencies, we have an in-house pharmacy that allows our clinicians to closely monitor the patient’s response to IV medication.

The Weekend Warrior will apply all nursing skills in this role, including infusion, wound care, ostomy teach, medication teach, and disease management. We encourage work-life balance through the ability to document at the point of care. Every Weekend, Saturday and Sunday

Saturday and Sunday, consistent weekend schedule

Visit window: 8:00 AM - 5:00 PM (Visits scheduled within this timeframe — not continuous bedside shifts)

Manageable caseloads with strong office and clinical support

You'll carry a caseload of home health patients across the valley, delivering skilled nursing visits and managing patient-centered care from start of care through discharge. Perform skilled nursing visits (wound care, wound vacs, IV therapy, medication administration)

Conducting Start of Care (SOC), Resumption of Care (ROC), recertification, and discharge assessments

Develop and update Plans of Care in collaboration with physicians

Coordinate care with interdisciplinary teams (LPNs, CNAs, therapy, pharmacy)

Active Arizona RN license (Compact RN license accepted)

Home health experience required (2+ years of direct patient care strongly preferred)

Graduate of an accredited nursing program (ADN or BSN accepted)

Current CPR/BLS and First Aid certification

Valid Arizona Level One Fingerprint Clearance Card (standard for home health in AZ)

Valid AZ driver's license, reliable transportation, and proof of auto insurance

Home infusion therapy

Wound care & wound vacs

EMR / point-of-care charting

2-3 years in a range of nursing roles
