Physical Therapist (PT) - Outpatient Therapy Clinics

Outpatient Physical Therapist
This position operates on a 5x8.0 schedule.
This is a 13-week contract position.
The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with patient needs, goals, and physician direction. This role is responsible for delegating and directing all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that allow for an effective return to community activities. The therapist serves as an educator for patients, families, students, coworkers, and themselves while performing other professional duties associated with patient care, development of discipline-specific therapy services, and program delivery.
Responsibilities:

Completes comprehensive evaluations of assigned patients and utilizes clinical reasoning to develop treatment plans focused on progressing patients toward discharge within an established length of stay.
Documents medical records clearly, accurately, and in a timely manner, inclusive of goal writing and charge entry in compliance with guidelines and clinic policies.
Participates in the education and supervision of students assigned to the clinic.
Actively pursues professional development through continuing education and self-study.
Maintains professional licensure in good standing and adheres to discipline-specific regulations and ethical standards.
Understands and participates in organizational performance improvement initiatives.
Required Experience / Certifications / Licensure:

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state licensure as a Physical Therapist.
• Previous experience with the agency is highly desired.
