Key Responsibilities

Evaluate patients and develop individualized physical therapy treatment plans within a skilled nursing facility setting

Administer therapeutic interventions to improve patient mobility, strength, and functional independence

Monitor and document patient progress, adjusting treatment plans as clinically appropriate

Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning

Educate patients and family members on home exercise programs and safe functional techniques

Required Qualifications

Active Physical Therapist (PT) license in the state of Minnesota or eligibility to obtain licensure

Graduate of an accredited Physical Therapy program

Experience working in a skilled nursing facility (SNF) or long-term care setting

Strong clinical documentation skills and knowledge of regulatory compliance standards

Preferred Experience / Skills

Prior contract or travel therapy experience

Familiarity with MDS documentation and PDPM reimbursement models
