Hospice Home Care Nurse (RN) - Nights

RN PD Nights 8pm - 8am or RN PD Days 8am - 8pmThe RN is responsible for the planning and delivery of patient care, providing skilled nursing care and oversight in accordance with Treasure Coast Hospice policies and the Florida Nurse Practice Act, utilizing the nursing process of assessment, planning, intervention, implementation and evaluation. To provide a combination of skilled professional nursing tasks and provide palliative and supportive care to the patient and family unit. phone triage (call center) and in-person visits (responder), crisis care and provides coverage to the inpatient units as needed and assigned. Provides care in Martin, St Lucie and Okeechobee counties.QualificationsCurrent and active Florida RN license.Must be Certified Hospice and Palliative Registered Nurse (CHPN) within 12 months.Minimum of one (1) year RN experience with acute care experience, preferably in ER or as a Charge Nurse.Prior experience in oncology, home health, Hospice or palliative care setting is preferred.Must be self-directed and able to work independently in the field.Ability to read, write and speak English. Bilingual preferred.Excellent communication and interpersonal skillsBasic computer skills and EMR software experience preferred.ExpectationsProvide program development strategies for Treasure Coast Hospice, in an organized, routine capacity that will enable the cohesion of team developments.
