Job Summary

The Orthopaedic Patient Navigator will function as a member of the Erlanger Orthopaedic team and manage and coordinate all aspects of care for patients with Orthopaedic disorders. The Navigator will work with Surgeons, hospital staff, office staff, and patients and their families. They will collaborate with the Orthopaedic Surgeons and other interdisciplinary team members to organize care delivery, facilitate the flow of patients through the system, and serve as the patient’s advocate. The Navigator will identify and address barriers to care that might prevent patients from receiving timely and appropriate treatment before and after surgery. Prior to surgery, the Navigator reviews pre‑testing screen and coordinates issues with the surgeon and others. During inpatient care the Navigator conducts daily rounds and coordinates post‑operative care, including 90‑day follow‑up. The Navigator is responsible for patient education, development of patient pathways and educational materials, tracking quality metrics, and implementing interventions to meet those metrics. They also help lead the Orthopaedic team in obtaining and maintaining specialty certifications and participate in community outreach. The Navigator is expected to function autonomously and demonstrate professional behavior when interacting with patients, community members, and the healthcare team.

Education

Graduate from an accredited School of Nursing. (Required)

Bachelor of Science Degree in Nursing from an accredited school in the State of Tennessee. (Preferred)

Experience

Minimum of 3 years of clinical experience. (Preferred)

Outpatient/Inpatient Orthopaedic care, prior educator, or navigation experience.

Quality improvement and/or discharge planning experience.

Greater than 2 years of Orthopaedic experience.

Position Requirements

Current nursing licensure to practice in the state of Tennessee as a Registered Nurse.

BLS completed prior to going to the unit/department and maintained thereafter.

Completion of 8 hours of Erlanger online education specific to Total Ankle, Hip, and Knee Replacement upon hire.

Preferred Qualifications

Bachelors of Science Degree in Nursing from an accredited school in the State of Tennessee.

Minimum of 3 years clinical experience.

Outpatient/Inpatient Orthopaedic care, prior educator, or navigation experience.

Quality improvement and/or discharge planning experience.

Greater than 2 years Orthopaedic experience.

Department Position Summary

The Orthopaedic Patient Navigator is a Bachelor or Masters prepared Registered Nurse or related discipline responsible for ensuring coordinated, seamless, and comprehensive quality services throughout the disease continuum. The Navigator functions on the multidisciplinary team as an advocate, interpreter, educator, clinical liaison, counselor, and collaborator for patients and their caregivers. The Navigator will demonstrate the ability to be self‑motivated with strong interpersonal skills, interact with all levels of health care providers and staff, and assist in patient education both before surgery and 90‑days after surgery. The Navigator will act as a liaison within the facility and in the community, promote awareness of services provided by Erlanger Orthopaedics, and collect and monitor quality metrics. The Navigator will report these metrics to leadership and surgeons, ensuring that evidence‑based and compliance standards are met. The Navigator will maintain clinical expertise in the role, serve as a resource person for the leadership and interdisciplinary team, and uphold patient privacy and safety guidelines while delivering compassionate care.
