Speech-Language Pathologist (SLP) Homeside Rehab

Homeside Rehab is seeking compassionate and dedicated Speech-Language Pathologists (SLPs) to provide individualized, evidence-based speech therapy services to patients throughout New York City's five boroughs . If you're passionate about helping patients improve their communication, language, feeding, and swallowing skills in a home-based setting, we'd love to hear from you!
Perform comprehensive speech-language evaluations and develop individualized treatment plans based on each patient's needs and goals.
Provide evidence-based therapy for speech, language, articulation, fluency, voice, feeding, and swallowing disorders, when applicable.
Deliver skilled speech therapy services in patients' homes while promoting functional communication and overall progress.
Collaborate with physicians, caregivers, teachers, and the interdisciplinary healthcare team to coordinate patient care.
Communicate effectively with office staff regarding scheduling, patient updates, and clinical needs.
Active New York State Speech-Language Pathologist License (Required)
Certificate of Clinical Competence (CCC-SLP) preferred, where applicable
Minimum 1 year of experience in Home Health, Outpatient, Early Intervention, or Skilled Nursing Facility (SNF) preferred
Ability to work independently in a home health setting
Basic computer proficiency for electronic documentation
Job Types: Full-time, Part-time, Per Diem
Compensation: Competitive hourly pay (based on experience)
Monday through Friday
