Required clinical activity in the last 24 months case logs Applicant must provide documentation of provision of gastroenterology services representative of the scope and complexity of the privileges requested during the previous 24 months applicants who completed training during the previous year must provide a training log.
AND If training to perform double balloon enteroscopy or capsule endoscopy occurred in the previous 2 years applicant must provide a description of supervised training on human subjects along with a letter from the supervising physician attesting to clinical competence as an independent operator. If training occurred greater than 2 years ago the applicant must provide documentation of ongoing clinical practice in each privilege requested.
Will provider work solo? No there are 2 other physicians in the department.

Required certifications Must be from AHA or American Red Cross as applicable: BLS and advanced GI training
