Brightway Living & Learning is a premier non-profit provider of human services on Long Island, including educational, residential, day habilitation, and vocational programs. Building on a 65-year legacy as the Developmental Disabilities Institute (DDI), Brightway supports over 1,600 children and adults with autism and other developmental disabilities through a full continuum of care. The Role: The LPN supports Brightway's residential programs and serves as a medical liaison for IDT team. The LPN updates the written plan for each person and maintains records for each individuals as well as appropriate in the planning and implementation of plan with Brightway personnel. NY State Licensed Practical Nurse required.

Develops written plan for each person with RN Supervisor. Modifies nursing plan as necessary.

Maintains medical records for each individual.

Participates as appropriate in planning and implementation of training of Brightway personnel.

Accompanies individuals to medical appointments as assigned.

Ensure proper medical follow-up through on call system.

Attends monthly Nursing Meetings.

Ensures compliance with OPWDD regulatory standards.

Liaison with family members regarding medical issues.

24-hour on call for medical emergencies with rotating weekend coverage required.

State Licensed Practical Nurse License required
