Title: Weekend Home Health RN (Field) Schedule: Ful Time (24 visits per week w/benefits) or Part Time (18 visits per week) Non-traditional schedules are available, i.e., Richmond, Rodeo, El Cerrito, El Sobrante, Hercules, Pinole, San Pablo, Crockett, Kensington AND Area #8 covers the areas of Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont Paid Training: Must be able to train for 5 days Mon-Friday 9am-5:30pm in San Ramon (onsite)

Position Summary: The Home Health RN monitors a patient's overall health, and in partnership with a primary care physician, develops an individualized plan of care for their patients. Each RN functions as a case manager, and works independently to deliver quality healthcare to their patients. As well as customized care and patient education about disease management, the Home Health RNs provide a variety of specialized services, such as Simple to Complex Wound Therapy, Post-Surgical Care Management, and Ostomy Care Management.

Minimum 1 year RN experience preferably in acute, SNF, Home Health or Hospice setting

Must Possess an active CA Registered Nursing License
