Licensed Practical Nurse - Primary Care Clinic - Full Time

Under the direction of the primary care operations team, the Primary Care Nurse works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way. The Primary Care Nurse will partner with patients, providers, and other members of the clinical care team to drive quality outcomes for an attributed panel. Schedule: Monday Friday, 0800-1630
No nights, weekends, or holidays!
**Must complete a minimum of three months of on-site training prior to transitioning to a hybrid work-from-home schedule. After training, this role follows a 50/50 hybrid model, with full-time (1.0 FTE) co-workers expected to work on-site five days per pay period. Must also be available to attend monthly on-site clinic meetings.**
Our Primary Care Nurse is a newly created role for the clinics. In this position the nurse will work closely with the Providers and manage triage calls/messages for the clinic. They will have the option to move hybrid after training is complete.

LPN

Qualifications:
Diploma in Practical Nursing.
Active LPN license.
RN Qualifications:
Associate of Applied Science in Registered Nursing, Graduation from registered nursing program, bachelor's degree in nursing (BSN), OR 4 years of commensurate experience in lieu of the bachelors degree.
Active RN license.
Proficiency with Epic EMR system
Computer competency (most of this position will be computer work)
Tuition Reimbursement up to $2,000 for continuing education
