Consonus Pharmacy is a closed-door long-term care pharmacy specializing in the needs of nursing facilities, assisted living residents, and their caregivers. We are currently hiring a Consultant Pharmacist to join our team that lives in the Eugene or Medford area!
Review resident medication regimens monthly (or as agreed upon) per patient acuity level and report findings in writing to facility administrator, director of nursing and physician

Plan and present in-service education to other pharmacy staff and nursing facility employees as requested

Understand and implement all appropriate pharmacy policies & procedures

Participate in the development of emergency drug supply kits at each facility in conjunction with the medical director, provider pharmacy and nursing administration

Liaison between facility and pharmacy staff

Provide ongoing education directly or indirectly to residents preparing for discharge from facilities serviced

Knowledge of CMS regulations and interpretive guidelines

Attend quality assurance and other appropriate committee meetings as requested by facilities

Prepare quarterly quality assurance reports

Or Pharm D. degree from an accredited School of Pharmacy

Current Pharmacy license in the state(s) of practice

Ability to travel throughout a territory, including overnight stays as needed
