The Patient Navigator builds individual and community capacity through outreach, education, counseling, and advocacy. Main Duties & ResponsibilitiesProvide accurate information to clients about living with HIV/HepC or taking PrEP, including how to prevent transmission, the benefits and challenges of care and treatment, and available services and how to access them. Accompany patients living with HIV and/or HepC and those patients vulnerable to HIV and HepC to medical or social service appointments as needed.Help people understand their health condition(s) and develop strategies to improve their health and well-being.Assist clients with enrollment or re-enrollment in medical care, medical benefits programs and other benefits for which they are eligible.Skills and KnowledgeLocal travel required.Bilingual fluency (English/Spanish) strongly preferred.ExperienceA minimum of two years of work experience in health, social or community services preferred.J-18808-Ljbffr Erie Family Health Center
