We are seeking a PRN Certified Occupational Therapy Assistant (COTA) to join our team in Medford, Oregon at Avamere Three Fountains. This 117-bed skilled nursing community is part of a larger campus that includes a health club and swimming pool that can be used for therapy. Other amenities include a large therapy gym, therapy kitchen, break area and excellent SLP space with quiet office. Implements and executes an established patient treatment plan under the supervision of a licensed Occupational Therapist
Provides timely written documentation per facility and department requirements
Provides timely written documentation as required by insurance provider to meet licensing, certification, regulatory, reimbursement requirements
Supports and participates in department operations and development
Graduate of an approved Occupational Therapy Assistant curriculum
Must hold an active COTA license in the state of Oregon
In good standing with all regulatory agencies and licensing boards
Full knowledge of resident's rights
Must be able to read, write, speak, understand, and communicate in English.
Must speak, read, and write English fluently
