Occupational Therapist (OT) – Home Health

What You’ll Do As a Home Health Occupational Therapist , you will play a critical role in helping patients regain independence, improve daily function, and safely navigate life at home. Assess home environments to identify safety risks and recommend modifications or adaptive equipment

Develop individualized treatment plans with clear, measurable goals focused on improving independence and quality of life

Fabricate splints and train patients in the use of adaptive, prosthetic, and orthotic devices as appropriate

Care Coordination & Communication Document patient care accurately and timely within the electronic medical record (EMR)

Communicate plans of care with physicians and interdisciplinary team members

Leadership & Oversight Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations and company policy

When therapy is the sole skilled service, provide oversight of home health aide care as required

Requirements Graduate of an accredited Occupational Therapy program

Current registration with the National Board for Certification in Occupational Therapy (NBCOT)

Current state licensure as an Occupational Therapist

Minimum of one (1) year of clinical experience preferred (home health experience a plus)

Valid driver’s license, reliable transportation, and auto insurance
