Travel Physical Therapist (PT) Assisted Living Rehab

Physical Therapist – General, travel assignment in a long-term care setting (SNF/ICF) in Hilo, Hawaii. Start date 09/14/2026 for a 13-week contract. Weekly pay range: $Shift: Rotating; generally five days per week with potential weekend coverage. This role is in a skilled rehabilitation setting and requires Hawaii state licensure and professional practice.

- Skilled Nursing Facility / Long-Term Care

Provide rehabilitation services in a SNF/ICF setting, performing evaluations, reassessments, and treatments within licensure and scope of practice.

• Develop, implement, and monitor individualized treatment plans in collaboration with supervising therapist, physician, and interdisciplinary team.
• Educate residents, families, and caregivers regarding treatment plans and patient progress.
• record progress, changes in plan of care, and discharges in accordance with facility, state, federal, and payer requirements.
• May supervise therapy assistants and be prepared for possible floating to other units or facilities based on census and staffing needs.

Ability to work a five-day schedule with potential weekend day coverage

• Must arrive the Thursday before start to complete employee health requirements
• Training: orientation to the rehabilitation department, EMR documentation (Net Health Rehab Optima and Epic), and facility workflows provided
