Are you a compassionate Social Worker looking to make a meaningful impact in hospice care? Our healthcare partner is seeking dedicated professionals like you to join their team in providing exceptional care in the home setting.
Your expertise will guide patients and families through the final stages of illness, ensuring they receive the support and care they need.
Provide intermittent skilled social work visits and support
Collaborate with nursing staff for pain and symptom management
Offer counseling, spiritual support, and bereavement services
Utilize personal automobile for travel to patient locations
Oregon LMSW or CSWA licensure required
Healthcare Provider Level BLS required
Oregon LCSW preferred
Experience as a social worker in a hospital or medical setting preferred
