About the Role:

Providing direct patient care , including administering medications, wound care, and other treatments as prescribed by physicians

Monitoring patient vital signs and reporting any changes or concerns to the nursing staff

Assisting with daily living activities , such as bathing, dressing, and feeding patients

Maintaining accurate and up-to-date patient records and communicating effectively with healthcare professionals

Participating in quality improvement initiatives to improve patient outcomes and satisfaction

Requirements:

Current LVN licensure in the state of California

Minimum of 1 year of experience in a long-term care setting or equivalent experience

Strong communication and interpersonal skills , with the ability to work effectively with patients, families, and healthcare professionals

Ability to maintain accurate and up-to-date patient records , and to communicate effectively with healthcare professionals

Physical ability to lift, transfer, and reposition patients , and to perform other physical demands of the job
