In this role, you will plan, organize, direct, and coordinate home care services while utilizing your nursing expertise and community health experience to support patients and families.The RN builds upon available community resources to develop and direct services that meet the unique needs of individuals and families within their homes and communities.ResponsibilitiesDevelop and initiate the plan of care and assume responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative, and caregiver(s).Initiate proper preventative and rehabilitative nursing procedures and provide services that are ordered by the physician as indicated in the plan of care.Identify discharge planning needs as part of the care plan development and implement plans prior to patient discharge.Ensure that arrangements for equipment and other necessary items and services are available.Supervise Licensed Practical Nurses (LPNs), Licensed Vocational Nurses (LVNs), and/or Home Health Aides (HHAs).AdmissionsRequirementsActive CA RN licenseMinimum of one (1) year of direct patient care experience; preferably in the hospice settingPossess a current CPR certificationBe a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working orderWhy Bella Terra Home Health and Hospice?Bella Terra Home Health and Hospice is part of the Cornerstone Group, a network of approximately 75 home health and hospice agencies across the country.
