We are seeking a dedicated RN Case Manager to join our growing hospice team. This role offers autonomy, a supportive interdisciplinary team, and the chance to deliver care that truly matters. • As an RN Case Manager, you'll serve as the primary nurse and advocate for your patients, ensuring they receive compassionate, coordinated care throughout their hospice journey.

Manage and coordinate a caseload of hospice patients and families

Complete patient assessments, admissions, care planning, and ongoing evaluations

Develop individualized care plans that address physical, emotional, social, and spiritual needs

Collaborate closely with physicians, social workers, aides, chaplains, and other interdisciplinary team members

Conduct routine patient visits and adjust care plans as needs change

Supervise and support Home Health Aides in accordance with state, federal, and company guidelines

Graduate of an accredited Registered Nursing program

Active Registered Nurse (RN) license in the state of practice

Current CPR and First Aid Certification

Knowledge of quality assessment and quality assurance practices
