Shifts: Available for visits from 8:00 AM to 4:30 PM, Monday to Friday, with one weekend per month availability for weekend PT admits
Seven Healthcare are seeking an experienced Physical Therapist for an onsite contract role in Pensacola, FL. Key Responsibilities of the Physical Therapist
Perform evaluations and develop treatment plans based on physician orders/referrals
Conduct comprehensive patient evaluations using appropriate assessment tools
Review patient records for diagnosis, medical history, and precautions
Establish measurable goals and develop plans to achieve maximum patient independence
Re-assess treatment effectiveness and modify plans as necessary
Communicate patient needs and progress to the treatment team and stakeholders
Consult with medical providers regarding patient evaluation and progress
Recommend adaptive equipment to improve patient function
Maintain and submit documentation as required by regulation and company policy
Supervise Physical Therapist Assistants and Aides
Attend and participate in staff development programs and obtain continuing education
Requirements of the Physical Therapist
Florida Physical Therapy license
Experience in Home Health and Oasis proficiency
Proficiency in point care/home care home base EMR
Experience in post-operative joint care therapy
