Weekly Estimated Pay: $2,190 - $2,550
Shift Schedule: Monday-Friday, 8:00am-4:30pm
Shift Duration: 8 Hour Days
Shifts per Week: 5
On Call / Weekend Requirement: 1 weekday evening per week (4:30pm-8:00am) and 1 weekend per month (Friday 4:30pm-Monday 8:00am)
Mileage Reimbursement: Yes (between visits, from first to last patient)
Contract Duration: 13 weeks
Minimum 1 year of homecare / home health RN experience
Experience with OASIS assessments and HCHB (Homecare Homebase) documentation
Current Registered Nurse (RN) license in Ohio
Clinical proficiency in wound care, wound VAC, ostomy care, blood draws, PICC/IV care, Foley care, and medication management
Provide skilled nursing and field RN services to patients in their homes across Portage, Medina, and Summit Counties
Complete OASIS assessments and accurate documentation using HCHB to meet home health compliance standards
Deliver skilled nursing interventions including wound care, wound VAC management, ostomy care, blood draws, PICC/IV and Foley care, and medication administration
Educate patients and family caregivers on care plans, medication management, wound care, and disease management to support safe transitions and reduce readmissions
Participate in on-call rotation and scheduled weekend coverage as required
Maintain productivity expectations (30 points per week) while providing high-quality, patient-centered home health care
