Pharmacist Position
The Hemostasis and Thrombosis Center (HTC) is home to over 45 faculty and staff, engaged in the clinical, research, public health, outreach and education missions. The HTC is a multidisciplinary center, providing comprehensive care to adult, adolescent and pediatric patients.
This position is responsible for providing the services of a licensed pharmacist within the HTC. The pharmacist specializes in caring for patients of all ages with hemophilia and other bleeding disorders in an ambulatory/outpatient pharmacy setting. The pharmacist in this role actively participates in patient visits, provides direct patient care, and collaborates closely with prescribers and the interdisciplinary clinic team. This position also participates in prescription verification, dispensing, and other outpatient pharmacy functions to ensure the safe and accurate distribution of medications. Pharmacy or Pharm.State of Oregon Board of Pharmacy Pharmacist licensure.
Community pharmacy practice or inpatient pharmacy practice experience.
Proficiency in clinical drug therapy management (dosing, indications, contraindications, interactions, monitoring).
Knowledge of contemporary medication-use systems (order entry/verification, dispensing, inventory management, and regulatory compliance).
ability to work independently with minimal supervision.
State of Oregon Pharmacy Preceptor licensure.
Advanced pharmacy practice residency.
Experience in specialty pharmacy practice, including hemophilia/bleeding disorders either inpatient or outpatient.
Experience working in an ambulatory care or clinic-based pharmacy setting, particularly within a multidisciplinary care team.
