UNT Health Clinical Practice Group’s Center for Older Adults provides comprehensive care plans that help prevent and manage the chronic diseases and long-term illnesses that may accompany the aging process.

Advance Care Planning
The Licensed Vocational Nurse (LVN) provides nursing care and clinical support for patients in a healthcare setting. The LVN assists in the delivery of patient care for individuals who are ill, injured, or seeking preventive health services. Working under the supervision of a registered nurse, physician, or other licensed healthcare provider, the LVN performs clinical procedures, monitors patient conditions, and supports the healthcare team in implementing treatment plans.
Graduation from an accredited school of nursing and one (1) year of related experience; A valid Texas Licensed Vocational Nurse (LVN) license and a current CPR (BLS) certification are required.
Administers medications, injections, immunizations, or intravenous fluids as directed by a licensed healthcare provider and document administration in patient records.
Observes patients and report changes in condition, including adverse reactions to medications or treatments, to appropriate healthcare personnel.
Prepares patients for examinations or procedures and assists healthcare providers during clinical visits.
Provides basic nursing care and treatments such as wound care, dressing changes, specimen collection, and other routine procedures.
Assists with patient intake, including obtaining health histories and documenting information in electronic health records.
Responds to patient questions and determines appropriate assistance or referral.
Assembles, maintains, and uses medical equipment and supplies necessary for patient care.
Maintains accurate, timely, and confidential documentation of patient care activities.
Supports clinical operations by preparing examination rooms and ensuring a safe and clean patient care environment.
Works collaboratively with healthcare providers and staff to support patient care, preventive services, and health education.
Bilingual competency (English/Spanish). Previous experience preferably with a geriatric population including telephone communication, ambulatory care, familiarity with adult vaccine schedule, patient rights, and safety. Required License/Registration/Certifications
Graduation from an accredited school of nursing. A valid Texas nursing License and a current CPR (BLS) certificate are required.
Measure and record patients' vital signs, perform procedures and administer medications, injections and vaccines as ordered by clinician in the electronic health record. Assembles, maintains, and properly uses equipment in patient care delivery.Return patient calls, and performs telephone triage following standardized protocols and clinicians guidance.
Gauges patients overall health status updating clinicians as indicated. Provides oversight and guidance to medical assistants in collaboration with clinic supervisor/manager.
Follows all departmental and organizational policies and procedures with regard to patient safety, quality management, infection control, and human resources. Ensure appropriate legal and regulatory standards.
Deliver culturally competent care that respects each patient's individual beliefs.
Participate in process and quality improvement initiatives that aim to ensure patient safety and efficient clinical workflows.
Coordination of care with internal and external partners.
