Physiatrist- Telemedicine

Role: Physical Therapist providing personalized care to older adults in private residences and Senior Living Communities.
Responsibilities: Conduct evaluations, develop treatment plans, deliver evidence-based therapy to improve mobility and independence, document in EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and incorporate telehealth when appropriate.
Qualifications: Valid Physical Therapy license, degree from an accredited program, proficiency in computer and EMR systems, and current CPR certification.
Skills & Requirements: Ability to meet physical demands of in-home care, including lifting, repositioning, and assisting with transfers, driving to patient locations, and wearing PPE as needed.
