We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team.

As a Licensed Practical Nurse (LPN) on our evening shift, you will play a vital role in providing high-quality patient care to our residents. You will be responsible for assessing, planning, implementing, and evaluating individualized care plans to promote health, independence, and well-being. Your expertise will be essential in managing the daily care needs of our residents, collaborating with other healthcare professionals, and contributing to a positive and supportive work environment.

Provide skilled nursing care to residents, including assessing, planning, implementing, and evaluating individualized care plans

Administer medications and treatments as prescribed by physicians

Perform vital signs, monitor resident conditions, and report changes to the healthcare team

Conduct daily rounds to ensure residents are comfortable, safe, and receiving quality care

Collaborate with other healthcare professionals, including physicians, nurses, and therapists, to develop and implement comprehensive care plans

Maintain accurate and detailed records of resident care and treatment
