You'll provide personalized rehabilitative care in patients' homes, helping them improve mobility, regain strength, reduce pain, and achieve greater independence.What You'll DoAs a Home Health Physical Therapist, you will:Perform comprehensive initial evaluations to identify functional limitations and establish appropriate therapy goals.Develop and implement individualized plans of care designed to improve mobility, strength, balance, endurance, and overall function.Provide therapeutic interventions to relieve pain and prevent or minimize physical disability.Evaluate patient progress and adjust treatment plans to achieve the best possible outcomes.Educate patients, families, and caregivers on exercises, mobility techniques, fall prevention, safety, and continued rehabilitation.Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary care team.Apply infection control and universal precaution standards during patient care.Ensure continuity and quality of care through accurate and timely documentation.Maintain all clinical documentation within company guidelines.Promote the agency's philosophy, standards, and commitment to exceptional patient care.What We're Looking ForGraduate of an accredited Physical Therapy program.Current and valid Physical Therapist license in the state of practice.Successful completion of applicable state licensure requirements.One year of home health experience preferred.Current CPR certification.Valid driver's license.Current automobile insurance.Homecare Homebase (HCHB) experience preferred.Physical RequirementsAbility to stand, walk, sit, climb, crawl, squat, and kneel for extended periods as required to provide patient care.Ability to lift, carry, pull, or push up to 100 pounds.Benefits & PerksAs a full-time team member, you may be eligible for:Medical, Dental & Vision Insurance401(k) Retirement Plan with Company Match Eligibility after 3 months of service100% immediately vestedChoice Health at Home matches 100% of the first 3% you contribute each pay periodHealth Savings Account (HSA)Life InsuranceShort-Term & Long-Term Disability InsurancePaid Time OffPaid HolidaysCompany Vehicle Opportunity – may be available after 90 days of full-time employment, based on vehicle availability and fulfillment of Fleet Car program requirements
