Hospice Care Liaison

Illuminate Hospice is the first and only provider specifically designed for skilled nursing patients and their care teams. Our mission is to provide compassionate care, ensuring dignity, respect, and quality of life for our patients while supporting their families and care providers. Patients and their loved ones can trust our team to meet their unique needs with compassion and expertise, fostering a sense of trust and comfort. The Hospice Liaison is responsible for driving census growth by developing and maintaining referral relationships with skilled nursing facilities, assisted living facilities, hospitals, physicians, and other community partners. This role serves as the primary relationship manager between Illuminate Hospice and its referral sources and ensures a strong, ethical, and compliant presence in the community.

Business Development & Growth
Skilled Nursing Facilities (SNFs)
Assisted Living Facilities
Physicians and community providers
Conduct in-services, educational sessions, and facility meetings
Represent Illuminate Hospice at community events and professional functions
Identify new growth opportunities and underperforming accounts
Serve as first-line contact for referral sources
Assist with collection of referral documentation as needed
Account Management & Facility Relations
Maintain regular in-person facility presence
Manage and grow assigned territory and accounts
Participate in care conferences and facility meetings as appropriate
Education & Compliance
Maintain strict compliance with:
CMS regulations
Company compliance policies
Document all:
Facility visits
Maintain CRM or referral tracking system
Submit weekly and monthly activity and performance reports
Attend census, marketing, and operations meetings, as requested
Participate in performance improvement initiatives
Performance Expectations
Excellent communication and relationship-building skills
Computer Experience: Word, PowerPoint, Excel, Outlook
Starting Salary: $85k plus bonus structure (based on experience)
