Registered Nurse – Home Health
Schedule: 8-hour shifts
Assignment Details:

Contract Length: 13 weeks
Guaranteed Hours: 40 hours per week
Requirements:

• Current state license as a Registered Nurse
• Homecare Homebase Software Experience
• OASIS Experience
• 1-2 years experience as a Registered Nurse in a clinical care setting
• 2+ years of home health experience
• CHPN certification (preferred)
• Proof of current CPR certification
• Current driver's license
• Reliable transportation and insurance

Assess patients in stable and emergent situations to identify their physical, psychosocial, and environmental needs.
Develop, document, and evaluate the Plan of Care (POC) to ensure high-quality, continuous care.
Educate patients and family members regarding care plans, medications, and safety practices.
Supervise and provide clinical direction to home health aides, ensuring quality care delivery.
Address pain management and educate patients and families on techniques to manage pain effectively.
Communicate effectively with patients, families, and interdisciplinary team members to coordinate care.
Compliance / Notes:

On-call responsibilities may be required.
Orientation and compliance checks may be mandated.
