Hospice Sales Liaison - Full Time

Reliant at Home is seeking a Full-Time Hospice Clinical Liaison in Denton, TX!
Hospice Sales experience required!
Reliant at Home is a multi-site, Texas only Home Health, Hospice, Caregivers, and Rehab company with 14 locations in Texas - including five Hospice locations (Fort Worth, Plano, Teague, The Woodlands, and San Antonio). The Hospice Clinical Liaison plays a dual role that combines clinical nursing expertise with strategic relationship-building. Serving as both an admissions nurse and a community liaison, this role is responsible for assessing patients for hospice eligibility, facilitating timely and compassionate admissions, and developing strong partnerships with referral sources to grow awareness and trust in our hospice services.
Ensure all admissions documentation is completed accurately, timely, and in compliance with federal, state, and agency standards.
Maintain up-to-date knowledge of hospice guidelines, insurance policies, and community resources.
Build and maintain strong relationships with hospitals, skilled nursing facilities, physicians, and community healthcare partners.
Represent the organization in the community at health fairs, professional networking events, and referral meetings.
Track and report referral patterns and outcomes, providing insight to leadership on trends and outreach opportunities.

Graduate of an accredited nursing program (Associate’s or Bachelor’s degree).
Previous experience in a clinical liaison, admissions, or healthcare sales role strongly preferred.
Proficient in electronic medical records (EMR) and documentation practices. Valid driver’s license, reliable transportation, and current auto insurance.
Flexibility to travel locally and work a schedule that meets the needs of referral sources and patients.
Ability to lift up to 50 lbs and manage physical requirements typical of in‑home or facility‑based clinical evaluations.
