Are you a compassionate Registered Nurse with a passion for diabetes education? If so, we have an exciting opportunity for you to join our healthcare partner in Rockford, IL. As a Diabetes Educator, you will provide holistic nursing care and education to patients and families living with diabetes mellitus, supporting them in managing their condition effectively.

In this role, you will collaborate with a multidisciplinary diabetes management team to deliver high-quality care and education. You will utilize your clinical expertise to guide patients through their diabetes journey, ensuring they receive the support and knowledge they need to manage their health. Utilizing the nursing process and collaborating with interdisciplinary team members in caring for patients

Administering prescribed medications and treatments in accordance with approved nursing techniques and standards of care

Documenting patient care according to standards of care

Developing, initiating, and executing individualized patient care plans

Participating in department or unit performance improvement activities

Providing clinical expertise and guidance to other units/departments

Acting as a resource for patient education program/materials development

Advocating for patients regarding clinical, psychosocial, and financial issues
