Shift:
Assess patient conditions and needs through comprehensive evaluations to develop individualized care plans.

• Develop and implement patient care plans in collaboration with the interdisciplinary team, ensuring comprehensive and continuous care.
• Monitor and evaluate patient progress continuously, adjusting care plans as necessary to ensure optimal outcomes.
• Document patient care activities meticulously to maintain accurate and up-to-date medical records.
• Educate patients and families about health conditions, treatments, and post-discharge care to promote understanding and compliance.
• Advocate for patient safety by identifying potential risks and implementing preventive measures.
• Collaborate with healthcare team members to ensure coordinated and effective patient care.
• Perform therapeutic and diagnostic interventions such as IVs, wound care, medication administration, catheterization, and specimen collections.
• Engage in quality improvement initiatives to enhance patient care quality, safety, and efficiency.
• Works independently, self-motivated, takes direction from physician and management to complete assignments. [Maintain safe knowledge of equipment, procedures, supplies, computer, IV pumps, NOVA Glucose monitor, I-STAT, Immediate use sterilization policy and procedures. [Must be proficient in English [Required]
