Coordinate nutritional services for residents, including planning and implementing individualized nutrition plans

• Complete and document nutrition assessments, monitoring residents' progress and adjusting plans as needed
• Participate in the resident care planning process, collaborating with interdisciplinary teams to ensure comprehensive care
• Provide nutritional education and counseling to residents and their families, empowering them to take an active role in their health and wellness
• Demonstrate a commitment to excellent customer service, prioritizing resident satisfaction and well-being

New York State Registered Dietitian certification

• Rehab experience a plus, but not required
• Excellent customer service skills, with a focus on delivering personalized care and support
