Physical Therapist Rehab (Full Time)
Personal Touch Home Health Care Services Inc.
Role: Physical Therapist in outpatient rehabilitation, focused on improving patient outcomes through hands-on care, education, and prescribed movement. Collaborates with a team of therapists and assistants to treat diverse patients of all ages.
Responsibilities Evaluate patients, develop and implement treatment plans, and reassess progress regularly.
Educate patients and families on home programs and preventive measures.
Prepare timely documentation and communicate with healthcare providers about patient care.
Maintain departmental policies, participate in educational activities, and contribute to special projects.
Qualifications & Skills Bachelor’s degree required; Doctor of Physical Therapy preferred.
NM Physical Therapy license required.
BLS certification at hire.
New graduates encouraged to apply.
