Physical Therapist - Outpatient
This is a full-time position with a flexible schedule, requiring 40 hours per week.
The Physical Therapist is responsible for evaluating assigned patients, developing, implementing, and modifying treatment plans in accordance with patient needs, goals, and physician direction. The therapist will ensure effective design of functional treatment programs, allowing patients to return to community activities. This role includes serving as an educator for patients, families, students, coworkers, and self, while upholding the organization's mission and core values.
Responsibilities

Conduct comprehensive evaluations of assigned patients.
Develop and implement individualized treatment plans and goals, progressing towards patient discharge.
Document medical records accurately and in a timely manner, ensuring compliance with guidelines and policies.
Supervise and mentor students as assigned, fostering a positive learning environment.
Maintain licensure in good standing and adhere to ethical practices in accordance with professional codes of conduct.
Participate actively in professional development and performance improvement initiatives.
Required Experience / Certifications / Licensure

A degree in Physical Therapy from an accredited school.
Minimum of 6 months of outpatient clinical experience.
Current CPR certification.
