This role offers the opportunity to work independently, manage complex medical conditions, and build strong, trusted clinical relationships — all while being supported by a collaborative interdisciplinary team.You will serve as a consistent clinical presence within your assigned facilities, providing proactive, patient-centered care and responding to urgent medical needs to ensure continuity, quality, and dignity in treatment.Roles and ResponsibilitiesProvide primary, urgent, and preventive medical care to residents in skilled nursing and, as applicable, assisted living settings.Independently evaluate, diagnose, and treat acute and chronic medical conditions.Monitor changes in resident condition and adjust treatment plans accordingly.prescribe medications and therapies within scope of practice.Collaborate with facility Medical Directors, attending physicians, nursing leadership, and interdisciplinary teams.Communicate clearly and compassionately with residents and families regarding diagnoses, treatment plans, and changes in condition.Maintain accurate and timely documentation in the electronic medical record in accordance with regulatory and payer standards.Participate in quality and care coordination efforts to support excellent patient outcomes.Minimum QualificationsActive, unrestricted Nurse Practitioner license in the applicable state(s) of practice.Current national Nurse Practitioner certification (ANCC, AANP, or equivalent).Clinical training and experience in adult, family, and/or geriatric medicine.Strong relationships drive strong care and overcome adversity together.Team Player: We support each other, bring all the facilities resources together, communicate openly, and solve problems together.Tenacity: Our passion for service creates an intensity to serve with you!
